

Round vs. Teardrop Implants: Which Shape Gives The Best Results?
When choosing breast implants, one of the most fundamental decisions is shape: round or teardrop. As a board-certified plastic surgeon in Houston, I am often asked which is better. After years of experience and a deep commitment to patient safety and satisfaction, my professional preference is clear.

What's the Difference Between Round and Teardrop-Shaped Implants?
Before we delve into why I prefer one over the other, it's important to understand the fundamental design of each implant type. The choice between them impacts the final silhouette, feel, and long-term stability of your breast augmentation results.
Round Breast Implants
These implants are perfectly symmetrical and circular. They provide uniform fullness across the entire breast, both in the upper and lower portions. This creates a balanced, lifted appearance and is especially effective at adding volume to the upper part of the breast, often called the “upper pole.”
Teardrop (Shaped) Breast Implants
Also known as anatomical implants, these are designed to mimic the natural slope of a breast. They are tapered at the top and fuller at the bottom. The goal of a teardrop implant is to create a result that is perceived as more “natural,” with less volume in the upper pole.
Which Shape Do I Recommend for My Patients?
In my practice, the conversation about implant shape is straightforward. I want my patients to have beautiful, lasting results that align with their goals and minimize potential complications.
For these reasons, I prefer using round implants.


Achieving Beautiful Upper Pole Fullness
The primary aesthetic goal for the vast majority of my patients is enhanced volume and shape. Most of my patients that come in want to have a lot of upper pole fullness in the breast. They want to look great in bathing suits, v-neck tops, and other clothing where that upper curve is most visible.
Round implants are specifically designed to deliver this result. Their symmetrical shape provides that desirable lift and fullness at the top of the breast.
In contrast, teardrop implants are designed to be flat on top. While marketed as “natural,” they often fail to meet the aesthetic expectations of patients seeking that enhanced silhouette. The shaped implant doesn't have a whole lot of that upper breast cleavage, and my patients are usually looking for that.
Eliminating the Risk of Implant Rotation
This is perhaps the most critical factor from a surgical and safety standpoint. A breast implant is placed within a “pocket” of tissue created during surgery. Over time, this implant can move or spin within that pocket.
With a round implant, this is not a problem. When I put a round implant in, the implant can spin and there's not going to be any problems. Because it's symmetrical, its appearance is unchanged no matter its orientation. This provides incredible peace of mind and long-term stability.
The same cannot be said for teardrop implants. Because they are asymmetrical, their orientation is crucial. If you use a shaped implant and that implant turns to the side, then your breast is going to look misshapen and you're not going to like the shape of it at all.
This complication, known as malrotation, creates a visibly distorted and unnatural breast shape. Worse yet, it is an extremely challenging problem to fix surgically once it occurs.


The “Natural” Look Is Often Indistinguishable
The main selling point for teardrop implants is their “natural” shape. However, compelling evidence suggests this benefit is minimal to non-existent once the implant is placed inside the body.
In a key study, a group of board-certified plastic surgeons was shown photos of patients—some with round implants and some with shaped implants. The surgeons were then asked to identify which implant each patient had.
The results were telling: The conclusion of the study was that even board-certified plastic surgeons could not look at each patient and reliably choose the right implant that the patient had in place.
If trained experts cannot consistently tell the difference, it calls into question whether the supposed aesthetic advantage of a teardrop implant is worth the very real risk of malrotation.
Round Implants Have Better Risk-Reward
As a surgeon, my primary responsibility is to guide patients toward the safest and most effective options. When it comes to round versus teardrop implants, the analysis is clear.
Teardrop implants introduce a significant risk of a disfiguring complication (rotation) that is difficult to correct, all for a supposed aesthetic benefit that is often imperceptible.
“In my opinion the risk is too high and the benefit is much, much, much too low.”
By exclusively using round implants, I can confidently provide my Houston breast augmentation patients with the beautiful upper pole fullness they desire, without the unnecessary risk associated with shaped implants.
